29 CFR 1910.106(e)(6)(i):Adequate precautions were not taken to prevent the ignition of flammable vapors: A.) Parker-LORD Corporation, Urethane Room 2, on or about 10/28/24 and times prior thereto: a flash fire occurred when a pelletized resin was added through an open manway in the 1000-gallon pigment tank that contained flammable liquids. Methods, techniques, and procedures developed for pigment tank mixing operations did not adequately prevent the ignition of flammable vapors ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.136(a): The employer did not ensure that each affected employee uses protective footwear when working in areas where there is a danger of foot injuries due to falling or rolling objects, or objects piercing the sole, or when the use of protective footwear will protect the affected employee from an electrical hazard, such as a static-discharge or electric-shock hazard, that remains after the employer takes other necessary protective measures: A.) Parker-LORD Corporation, Urethane Room 2, on or about 10/28/24 and times prior thereto: employees were not required to utilize protective footwear including, but not limited to, static dissipative footwear, when manually adding pelletized resin through an open manway in the 1000-gallon pigment tank which contained flammable liquids. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
