On or before July 20, 2022, at a worksite located at 134 Chapman Drive, Lot 324, Pataskala, Ohio 43062: The employer failed to ensure employees used electrical equipment where the path to ground was permanent and continuous. Employees used an extension cord that was missing the ground prong, thereby being exposed to a 123-volt electrical contact hazard.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.  a. On or before July 20, 2022, at a worksite located at 134 Chapman Drive, Lot 324, Pataskala, Ohio 43062: the employer failed to ensure strain relief was provided to cords so that the pull was not directly transmitted to joints or terminal screws. Employees used an air compressor cord where the plug of the cord had separated from the jacket of the cord, thereby exposing employees to a 123-volt electrical contact hazard.

29 CFR  1926.20(b)(1):It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.  This employer continues to fail to ensure employees are protected from common hazards found in residential construction by initiating and maintaining safety programs while framing a new build residential house.  The most recent incident of such failure occurred on or before July 20, 2022, at a worksite located at 134 Chapman Drive, Lot 324, Pataskala, Ohio 43062:  the employer failed to initiate and maintain an effective safety and health program to ensure that hazards were identified and corrected onsite while employees performed framing work.   Clarence M. Brigner III was previously cited for a Willful violation of this Occupational Safety and Health standard 29 CFR 1926.20(b)(1) which was contained in OSHA inspection number 1544329, citation number 1, item number 1 and was affirmed as a final order on 9/28/2021, with respect to a workplace located at 115 Beaman Gates Drive, Lot 273, Pataskala, Ohio 43062.  Clarence M. Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   This employer continues to fail to ensure employees are protected from falls while framing the roof of a new build residential home.   The most recent incident of such failure occurred on or before July 20, 2022, at a worksite located at 134 Chapman Drive, Lot 324, Pataskala, Ohio 43062:  the employer failed to ensure that an employee was protected from falls while conducting framing work on the roof of a new build residential home. The employee was exposed to a fall distance of 20 feet to the ground level.  Clarence M. This employer continues to fail to ensure employees are trained on the recognition of  fall hazards and the procedures to be followed in order to minimize these hazards while framing a new build residential house.  The most recent incident of such failure occurred on or before July 20, 2022, at a worksite located at 134 Chapman Drive, Lot 324, Pataskala, Ohio 43062:  the employer failed to train employees on the common fall hazards found in residential construction for employees performing framing work.
