When wet processes are used, drainage must be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided. a) On or about July 20, 2022, employees were exposed to wet floors with water and chemicals including, but not limited to SODIUM CYANIDE, SODIUM HYDROXIDE SOLUTION (50%), CAUSTIC SODA BEADS and SULFURIC ACID 66 DEGREE during the metal plating process located at 1028 East 134th Street, Cleveland, Ohio 44110; the employer did not ensure that employees were protected from a slip hazard during wet processes by use of drainage, false floors, platforms, or mats. b) On or about July 20, 2022, employees were exposed to wet floors with water and chemicals including, but not limited to SODIUM CYANIDE, SODIUM HYDROXIDE SOLUTION (50%), CAUSTIC SODA BEADS and SULFURIC ACID 66 DEGREE during the waste treatment process located at 1028 East 134th Street, Cleveland, Ohio 44110; the employer did not ensure that employees were protected from a slip hazard during wet processes by use of drainage, false floors, platforms, or mats.

Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a) On or about July 20, 2022, employees were observed without the use of the SDS's recommended PPE where employees worked with SODIUM CYANIDE, SODIUM HYDROXIDE SOLUTION (50%), CAUSTIC SODA BEADS and SULFURIC ACID 66 DEGREE during the metal plating process located at 1028 East 134th Street, Cleveland, Ohio 44110; the employer does not protect employees from chemical burn hazards by ensuring the use of PPE (apron) during the metal plating process. b) On or about July 11, 2022, employees were observed without the use of the SDS's recommended PPE where employees worked with SODIUM CYANIDE, SODIUM HYDROXIDE SOLUTION (50%), CAUSTIC SODA BEADS and SULFURIC ACID 66 DEGREE during the waste treatment process located at 1028 East 134th Street, Cleveland, Ohio 44110; the employer does not protect employees from chemical burn hazards by ensuring the use of PPE (apron) during the waste treatment process resulting in an employee injury.

Soldered splices shall first be spliced or joined to be mechanically and electrically secure without solder and then soldered. All splices and joints and the free ends of conductors shall be covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose. a) On or about November 30,2022, employees were exposed to a shock hazard from approximately 480- volts while using the toggle switch for the blue fan in the maintenance area; the employer did not ensure the splices in the SOOW cord conductors above the toggle switch were covered with an insulation equivalent to that of the conductors or with an insulating device identified for that purpose. b) On or about September 28, 2022, an employee was exposed to a shock hazard from approximately 480-volts while moving the 50 horse power Wenniger blue compressor near the zinc line; the employer did not ensure the multiple splices in the wire conductors above the electrical panels were covered with an insulation equivalent to that of the conductors or with an insulating device identified for that purpose.
