29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: At a worksite located at 5964 S State Hwy NN, Rogersville MO 65742: Employees were exposed to electrical shock hazards due to the electrical outlet box being plugged into another set of extension cords in series and not in accordance with the listing, labeling, and certification of the extension cords. On or about February 9, 2022, an electrical outlet box was being used by employees to power a Bosch table saw for the purpose of cutting engineered plastic 2x6 boards. The outlet box was powered by two extension cords that were plugged together in series and run back an electrical wall receptacle.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. At a worksite located at 5964 S State Hwy NN, Rogersville MO 65742: On or about February 9, 2022, a flexible cord and cable (extension cord) were being used to power an outlet box that was connected in series to a Bosch table saw. The flexible cord and cable was missing a ground pin and exposed employees to electrical shock hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: At a worksite located at 5964 S State Hwy NN, Rogersville MO 65742: On or about February 9, 2022, flexible work and cable (extension cord) was plugged in series to an electrical outlet box was being used by employees to power a Bosch table saw. Employees were exposed to electrical shock hazards when the extension cord was experiencing strain relief while it was connected to live parts of electrical equipment.
