29 CFR 1926.453(b)(2)(iv):Employee(s) working in an aerial lift were not standing firmly on the floor of the basket: At a commercial worksite located at 3055 E Division St, Springfield MO -- an employee working from an aerial lift stepped on the 2nd rail and over the top rail of the aerial lift. This exposed the employee to fall hazards excess of 10 feet to ground level. This was documented on or about 10/19/2022.
