29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about July 21, 2022 the employer SCC Construction Inc. at jobsite located at 626 Lorain Blvd. Elyria OH 44035, did not ensure the side rails of one portable aluminum extension ladder were extended at least three feet above the landing surface. Five employees were exposed to falls from approximately 10 feet or greater from the top of the second (roof) level to the decking below while using the ladder to access the second (roof) level to perform commercial roofing activities on a church building.

29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.: On or about July 21, 2022 the employer SCC Construction Inc. at jobsite located at 626 Lorain Blvd. Elyria OH 44035, did not ensure employees were protected from falls while performing roofing activities atop a low sloped commercial structure. Five employees installing new roofing shingles to the church structure were exposed to fall hazards of 11 feet and greater to the ground below. SCC Construction Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 1926.501(b)(13), which was contained in OSHA inspection number 1558764, citation number one, item number one and was affirmed as a final order on January 16, 2022, with respect to a workplace located at 6510 Snow Rd. Parma OH 44129.
