(a) Facility at 280 Great Island Road in Narragansett:  On or about July 22, 2022, the employer had not established a program consisting of energy control procedures, employee training and inspections to ensure the startup or release of stored energy could not occur while operating and working on or in machinery such as the Semco Model SC 9910 Portable "Ice Plant" to include Semco ice storage bin, thereby exposing employees to caught-in and crushed by hazards.
