(a) Facility located at 280 Great Island Road in Narragansett, RI: On or about July 19, 2023, the employer did not inform exposed employees of the existence and location of the permit-required confined spaces, including but not limited to the Semco Ice Bin, by posting danger signs that state: "DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER".
