OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to respiratory hazards from an ammonia release:  a) Centennial Ice Arena, Mechanical Room:  On or about July 21, 2022 and at times prior, employees are exposed to ammonia release and subsequent fire hazards while performing system checks and maintenance without evaluating the integrity and proper operation of the components of the ammonia refrigeration system such as, but not limited to, compressors, vessels, piping, pressure relief valves and safety systems.   b) Centennial Ice Arena, Mechanical Room:  On or about July 21, 2022 and at times prior, employees are exposed to ammonia release and subsequent fire hazards while operating and maintaining ammonia refrigeration equipment without training regarding the safe operation of the equipment, including, but not limited to, schematic drawings, information about the basic operation and function of each component, information identifying normal operating pressure and temperature ranges of the system components, and indications of system malfunction.
