29 CFR 1910.272(k):Receiving pit feed openings in the grain handling facility, such as truck or railcar receiving pits, were not covered by grates which had a maximum opening width of 2 1/2 inches: Employees are exposed to safety hazards. Employees engaged in grain receiving operations within the south drive receiving area were exposed to safety hazards in that a single gap in the grate that covered the receiving pit measured 3 3/8" between two metal grate bars.
