29 CFR  1910.157(e)(3):The employer did not assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request:  On or about July 28, 2022, and at times prior thereto, employees were exposed to a fire hazard, while working in the shop, without the fire extinguishers subjected to an annual maintenance check.

29 CFR  1910.219(e)(1)(i): Where both runs of horizontal belts are seven (7) feet or less from the floor level, the guard did not extend to at least fifteen (15) inches above the belt or to a standard height, except that where both runs of a horizontal belt are 42 inches or less from the floor, the belt was not fully enclosed in accordance with paragraphs (m) and (o) of this section:   On or about July 28, 2022, and at times prior thereto, employees were exposed to amputation hazards while operating the post-tensioning machine (brand unknown) without the horizontal power transmission pulleys not being guarded.

29 CFR  1910.253(b)(2)(ii): Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders were not stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces were not located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:  On or about July 28, 2022, and at times prior thereto, employees were exposed to a fire hazard, while working in the shop, with oxygen and acetylene cylinders stored adjacent to each other.
