29 CFR  1910.94(a)(5)(v): Operators shall be equipped with heavy canvas or leather gloves and aprons or equivalent protection to protect them from the impact of abrasives. Safety shoes shall be worn to protect against foot injury where heavy pieces of work are handled.  At the facility located at 822 S 14th Street, Manitowoc, WI 54220, in Large Coil, the employer did not provide and require the use of heavy canvas gloves, leather gloves, or equivalent protection to protect employees from the impact of abrasives when performing abrasive blasting on equipment such as, but not limited to coils, exposing employees to struck by flying particles.

29 CFR  1910.132(d)(1)(i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment;  At the facility located at 822 S 14th Street, Manitowoc, WI 54220, in Large Coil, the employer did not select and require the use of personal protective equipment that will protect employees from the hazards identified while performing abrasive blasting, exposing employees to struck by flying particles. The employer did not require the use of heavy canvas gloves, leather gloves, or equivalent protection while performing abrasive blasting.

29 CFR  1910.132(d)(2): The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.  At the facility located at 822 S 14th Street, Manitowoc, WI 54220, the employer did not certify in writing that a hazard assessment had been performed to evaluate the workplace hazards and the personal protective equipment those hazards necessitated when employees were exposed to hazards while performing work in Small Coil, such as but not limited to spray coating and dipping.

29 CFR  1910.134(e)(1): The employer shall provide a medical evaluation to determine the employee's ability to use a respirator, before the employee is fit tested or required to use the respirator in the workplace. The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.  At the facility located at 822 S 14th Street, Manitowoc, WI 54220, the employer did not provide medical evaluations to determine each employee's abilities to use respirators, exposing employees to the hazards associated with inadequate physical/physiological capability. Employees in Large Coil and Small Coil were using supplied-air respirators or half-face elastomeric respirators to protect from respiratory hazards while performing work such as, but not limited to abrasive blasting and spray coating.
