29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a.  On or about July 28, 2022, at the above addressed jobsite, an employee using a pneumatic nail gun did not wear safety glasses, thereby exposing the employee to the hazards associated with eye injuries.

On or about July 28, 2022, at the above addressed jobsite, an employee was carrying roof shingles up the ladder, thereby exposing the employee to hazards associated with falls.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a.  On or about July 28, 2022, at the above addressed jobsite, employees performing residential roofing operations, eighteen (18) feet four (4) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.  b.  On or about July 28, 2022, at the above addressed jobsite, employees performing residential roofing operations, sixteen (16) feet four (4) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.  Fellows Roofing LTD. was previously cited for a violation of this occupational safety and health standard  1926.501(b)(13), which was contained in OSHA inspection number 1496446, citation number 1, item number 1 and was affirmed as a final order on December 31, 2020, with respect to a workplace located at 2024 E. Vargo, Arlington Heights, IL 60004.

On or about July 28, 2022, at the above addressed jobsite, employees had been provided fall protection training, however, the employer failed to keep a written certification of record.
