29 CFR  1910.178(l)(5): Avoidance of duplicative training. If an operator has previously received training in a topic specified in paragraph (l)(3) of this section, and such training is appropriate to the truck and working conditions encountered, additional training in that topic is not required if the operator has been evaluated and found competent to operate the truck safely.   a. On or about July 25, 2022, at the above address, an employee was required to operate a powered industrial vehicle and had not been provided with training, including but not limited to formal instruction, practical training and an evaluation of the operator's performance, thereby exposing employees to struck-by and crushing hazards.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  a. On or about July 25, 2022, at the above addressed jobsite, employees performing masonry cleaning operations from a man-made wooden box were not trained to recognize the hazards associated with scaffolding. The employees were thereby exposed to the hazards associated with falls.

On or about July 25, 2022, at the above addressed jobsite, the employer was using an unapproved attachment (a wooden job made box) on the forks of a power industrial vehicle/LULL 644B-42 forklift to hoist two (2) employees, thereby exposing employees to fall and struck-by hazards.

29 CFR  1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a. On or about July 25, 2022, at the above addressed jobsite, employees conducting masonry cleaning activities from a mobile scaffold work platform, approximately seventeen (17) feet above the ground, were exposed to fall hazards due to the lack of a complete guardrail system.  Camposs Construction, Llc was previously cited for a violation of this occupational safety and health standard 1926.451(g)(4)(i), which was contained in OSHA inspection number 1548009, citation number 1, item number 1 and was affirmed as a final order on January 6, 2022, with respect to a workplace located at 8411-19 S. Harlem Avenue in Bridgeview, IL 60455.
