29 CFR 1910.133(a)(3): The employer did not ensure that each affected employee who wore prescription lenses while engaged in operations that involved eye hazards wore eye protection that incorporated the prescription in its design, or wore eye protection that could be worn over the prescription lenses without disturbing the proper position of the prescription lenses or the protective lenses. a) Design Department - On or about July 27, 2022, the employer exposed employees to struck by hazards, in that employer failed to ensure that employees were provided with eye protections that could be worn over the prescription lenses when employees were cutting fabrics and wearing prescription glasses.

29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire: a) Welding Department - On or about July 27, 2022, the employer exposed employees to fire hazards, in that employer failed to ensure that oxygen cylinders were not stored near highly combustible materials such as, but not limited to, acetylene gas cylinders while employees were operating forklifts and working/walking at the facility.
