29 CFR  1926.451(f)(6): The clearance between scaffolds and insulated power lines less than 300 volts was not a minimum of 3 feet when scaffolds were erected or used such that they or any conductive material handled on them might come closer to exposed and energized power lines:  a) Jobsite located at or near 2227 Wilder St, Philadelphia, PA - On or about July 29, 2022, the employer did not ensure he clearance between scaffolds and energized, insulated power lines less than 300 volts was not a minimum of 3 feet when scaffolds were erected or used. Employees were jointing brick from a scaffold that was 21 inches from an insulated power line less than 300 volts, thereby exposing employees to electrical hazards.
