29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about July 25, 2022: Employees performing maintenance on the Icing Liquefier Machine at 4651 F St, Omaha, Nebraska, were exposed to amputation hazards when machinery with unguarded rotating parts was not effectively deenergized and locked out. One employee received an amputation injury while attempting to remove foreign material from a rotating pump impellor.
