On or about July 28, 2022, at an establishment located at 111 North Jackson Street in Valders, Wisconsin, in Plant 3; the employer did not require employees to use appropriate hand protection when employees' hands were exposed to severe laceration and amputation hazards created by handling the Sumiden wire strands.
