29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: 700 South Main Street, Walsenburg, CO: On or about August 3, 2022, and at times prior, the employer, By the Rio, LLC dba Carl's Jr., did not furnish a place of employment free from hazards. Employees were exposed to slip, trip, and fall hazards while entering and exiting the walk-in refrigerator. The walking surface of the walk-in refrigerator has a broken metal flooring with a one inch trip hazard.
