When wet process were used, mats were not provided.  a) Meat Department: On or about August 3, 2022, cardboard was on the floor used as a non-slip surface and no mats when employees were cutting meat, exposing's employees to slip, trips, and or fall hazards.

No materials or equipment may be placed, either permanently or temporarily, within the exit route.   a) Meat Department: On or about August 3, 2022, the exit door was blocked by a storage room door that was left open, exposing the employees to a blocked exit route, fire hazard, and delayed egress.

29 CFR  1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action or overheating.    a) Meat Cutting Room: On or About August 3, 2022, an energized electrical panel connected to a Hobart Meat Hopper had a plastic bag wrapped around the top conduit, exposing employees to electrical and fire hazards.  b) Meat Cutting Room: On or About August 3, 2022, an energized electrical panel connected to a Hobart Meat Grinder had a plastic bag wrapped around the top conduit, exposing employees to electrical and fire hazards.  c) Meat Cutting Room: On or About August 3, 2022, an energized electrical panel that links both the grinder and the hopper together, was broken with scotch tape holding it together, exposing employees to electrical hazards.  d) Meat Cutting Room: On or about August 3, 2022, a Hobart commercial meat saw had a single pull-to-start power switch, the switch does not stay in the 'On' position while in operations. This exposes employees to an unexpected start-up that could result in an amputation or laceration hazard.

Interior, front and rear column baseplates on the frames of the racking system were not anchored to the floor to resist all applicable forces, exposing employees to struck-by and crushing hazard.
