29 CFR 1915.74(a)(9): The employer permitted employees to board or leave a vessel when the upper end of the means of access rested on or was flush with the top of bulwark and substantial steps properly secured and equipped with at least one substantial handrail approximately 33 inches in height was not provided between the top of the bulwark and the deck. M/V Apache: On and/or before August 2, 2022 the gangway installed was missing substantial steps with at least one handrail. The distance from the gangway to the vessel deck was measured 29 inches in height. This condition potentially exposed employee(s) to the slip, trip, and fall hazard(s).
