29 CFR 1915.74(a)(1): The employer permitted employees to board or leave a vessel by using a gangway with less than 20 inches walking surface, of adequate strength, maintained in safe repair, and safely secured. M/V VICKY ANN, Gangway: On and/or before August 2, 2022, employees conducted refurbishment operations with heavy equipment transported onto upper deck and used a gangway with mid-rail bent and broken at attachment point. The employer did not maintain gangway in safe repair. Employees were potentially exposed to fall hazards.
