29 CFR 1926.451(c)(1):Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph. (a) 2500 South Abilene Street, Aurora, CO 80012: On or about August 5, 2022, The controlling employer, DDG Construction, Inc., observed sub-contracted employees of Fox Valley Construction Group, Inc., working on a supported scaffold with a height-to-base of more than 4:1 ratio. The employees were exposed to fall hazards due to the supported scaffold not being properly restrained from tipping by guying, tying, bracing or its equivalent.

29 CFR 1926.451(f)(3):Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: (a) 2500 South Abilene Street, Aurora, CO 80012: On or about August 5, 2022, the controlling employer, DDG Construction, Inc. as the competent person, did not conduct an inspection of the supported scaffold being used by sub-contractor, Fox Valley Construction Group, Inc., prior to any occurrence that could affect the structural integrity of the scaffold and the scaffold components. This exposed employees to fall hazards.
