29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe-boards, safety net systems or personal fall arrest systems:  (a) On or about 08/05/2022 - 21940 Shearwater Drive, Madison, AL, employees were exposed to fall hazards of approximately 9 feet while placing drip edge on a steep sloped (8:12) roofing system.  Matias Alonzo Martin was previously cited for a repeat-serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1383355, citation number 1, item number 2a and was affirmed as a final order on 06/07/2019, with respect to a workplace located at 121 Count Fleet Court, Lot 163, Madison, AL 35756.

Matias Alonzo Martin was previously cited for a repeat-serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1383355, citation number 1, item number 2b and was affirmed as a final order on 06/07/2019, with respect to a workplace located at 121 Count Fleet Court, Lot 163, Madison, AL 35756.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:   (a) On or about 08/05/2022 - 21940 Shearwater Drive, Madison AL, employees utilizing a Werner aluminum extension ladder were exposed to fall hazards of approximately 9 feet due to the ladder not being extended 3 feet or more above the landing surface.  Matias Alonzo Martin was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1300207, citation number 1, item number 3a and was affirmed as a final order on 04/06/2018, with respect to a workplace located at 4505 Cattail Cove SE, Owens Cross Roads, AL 35763.

29 CFR 1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards.  (a) On or about 08/05/2022 - 21940 Shearwater Drive, Madison, AL, employees using a Werner aluminum extension ladder to access the landing surface of a roofing system did not recognize the fall hazards associated with not setting up a ladder properly.  Matias Alonzo Martin was previously cited for a serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1060(a)(1), which was contained in OSHA inspection number 1300207, citation number 1, item number 3b and was affirmed as a final order on 04/06/2018, with respect to a workplace located a 4505 Cattail Cove SE, Owens Cross Roads, AL 35763.
