29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. At a worksite located at 2027 Stevens Street, in Jacksonville, Florida 32207:  On or about August 9, 2022, the employer exposed employees to head injury hazards, in that protective helmets (hard hats) were not utilized by employees working at ground level and beneath other employees working from the roof and truss areas, while performing framing operations.

29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. At a worksite located at 2027 Stevens Street, in Jacksonville, Florida 32207: On or about August 9, 2022, the employer exposed employees to eye injury hazards, in that eye protection was not utilized by employees who were operating pneumatic nail guns to nail down plywood and set trusses while performing framing operations.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On a 5:12 pitched roof, at a worksite located at 2027 Stevens Street, in Jacksonville, Florida 32207: The employer exposed employees to fall hazards ranging from 9 to 17-foot to the ground below, in that fall protection was not utilized by employees while performing framing operations, to include installing roof trusses and roof deck sheathing.

On the right side of the home located at 2027 Stevens Street, in Jacksonville, Florida 32207: On or about August 9, 2022, the employer exposed employees to fall hazards in that, the employer allowed employees to use a 6-foot portable A-Frame type ladder, in the closed position while performing framing operations.
