On or about March 04, 2025, at a residence located at 995 Caiden Dr. Ponte Vedra , FL 32081, the employer exposed employees to fall hazards, in that A frame ladders were not being utilized for the purpose for which they were designed in order to gain access to a residential roof. Gaalex Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(4) which was contained in OSHA inspection number 1613331, citation number 1, item number 4 and was affirmed as a final order on November 17, 2022, with respect to a workplace located at 2027 Stevens Street, in Jacksonville, Florida 32207.
