29 CFR 1926.501(c): When employees were exposed to falling objects the employer did not require employees to wear hard hats and did not implement measures to prevent objects from falling from higher levels as set forth in (c)(1)-(3) of this section: Location: 88 Grove St, Monsey, NY 10952 a) On or about August 3rd, 2022, the employer did not ensure employees were wearing hardhats while engaged in commercial construction from falling objects approximately 30ft or more above lower levels. b) An employee was working on a ladder and was struck-by a falling object on the head while not wearing a hardhat on the construction site.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: Location: 88 Grove St, Monsey, NY 10952 a) On or about August 3, 2022, an employee was installing pipe on a plumbing system approximately 9ft without an extension of at least 3ft to get on and off the landing surface.
