On or about August 5, 2022, in the Amines area, employees were exposed to thermal and chemical hazards when the employer did not provide written procedures for working on piping for the drain of Pump G 2416 while in service.

1910.119(j)(6)(ii) Appropriate checks and inspections were not performed to assure that equipment was installed properly and consistent with design specifications and the manufacturer's instructions. On or about August 5, 2022, in the Amines Ethanolamine unit, employees were exposed to hot triethanolamine and diethanolamine when a process release from the casting drain of Pump G2416 occurred. Inspection of the Pump G2416 casing drain nipple was not effectively implemented to verify that the correct material of construction was in place when the pump G2416 casing drain nipple was installed.
