29 CFR 1910.212(a)(2): Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible. At a worksite located at 8600 Northeast 38th Street, Kansas City, Missouri. Employees were exposed to a stuck by, amputation hazard when clearing jams from the scrap baler machine. The bottom guard was not affixed to the scrap baler machine or by other methods that offered full employee protection and was absent an interlock device.
