29 CFR  1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about August 11, 2022 the employer did not ensure that machine specific procedures for the control of hazardous energies were utilized during service and maintenance of the Komatsu 60 ton mechanical power press. Employees were performing service and setup on the equipment without the use of LO/TO. Energy sources on the equipment include but are not limited to gravitational, mechanical, and pneumatic.   b) On or about August 11, 2022 the employer did not ensure that machine specific procedures for the control of hazardous energies were utilized during service and maintenance of the Aida 110 ton mechanical power press. Employees were performing service and setup on the equipment without the use of LO/TO. Energy sources on the equipment include but are not limited to gravitational, mechanical, and pneumatic.   c) On or about August 11, 2022 the employer did not ensure that machine specific procedures for the control of hazardous energies were utilized during service and maintenance of the #19 Screw Machine. Employees were performing service (shave head replacement) and setup on the equipment without the use of LO/TO. Energy sources on the equipment include but are not limited to hydraulic, mechanical, and pneumatic.

29 CFR  1910.184(c)(6):Slings were not securely attached to their loads:  On or about August 11, 2022 the employer did not ensure that metal slings in use at the facility could be securely attached to their loads. The spring latch device of the load hook was broken and did not return. This allowed for the load to remain unsecured when lifting.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a) On or about August 11, 2022 the employer did not ensure that effective machine guarding was in place on the South Bend lathe located in the machine shop area. The lathe did not contain a chip/ chuck guard during operation.   b) On or about August 11, 2022 the employer did not ensure that effective machine guarding was in place to protect employees from moving machine parts on the screw machines located within the production area of the facility. Doors were not in place, opened during operations, or interlock mechanisms broken or in need of repair. Screw machines not effectively guarded at the time of  inspection and in operation include #7,11,12,18,19,21, and 26 (Conomatic Machine Co. screw machines)  c) On or August 11, 2022 the employer did not ensure that effective machine guarding was in place to protect employees from the moving machine components of the Abex Denison  Multipress (hydraulic press). The barrier guard in place on the press area was large enough that employees could reach into the hazardous area during operation.

29 CFR  1910.217(c)(1)(i):The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es):  On or about August 11, 2022 the employer did not ensure the use of effectively applied point of operation guarding on the Komatsu 150 mechanical power press. The press was ran in continuous and auto feed mode without a barrier guard in place. Additionally, the interlock for the front barrier guard did not function and was in need of repair.

29 CFR  1910.178(l)(4)(i):1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance shall be conducted at least once every three years.  On or about August 11, 2022 the employer did not ensure that employees had received refresher training for the safe use and operation of powered industrial fork trucks at the facility.
