On or about August 8, 2022, the lock out tag out procedure for the BuhlerPrince die cast machine #61 (MN 836CCA, SN DB7004) did not include verification that hazardous energy sources were isolated prior to servicing and maintenance of the machine by employees. Employees were exposed to amputation and crushing hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. On or about August 8, 2022, the employer did not perform periodic inspections on the energy control procedures, such as the BuhlerPrince die cast machine #61, including a review with each authorized employee responsibilities under the energy control procedure. Employees were exposed to amputation and crushing hazards. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: On or about August 8, 2022, employee(s) performing servicing and maintenance tasks such as set-up were exposed to bodily injury and crushing hazards due to unexpected start-up. Employees were not required to control hazardous energy sources in the following sequence. (1) prepare for shutdown, (2) shut down machine or equipment, (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source, (4) affix lockout or tagout devices to each energy isolating device by authorized employee, (5) relieve, disconnect, restrain or otherwise render safe all stored or residual energy, (6) verify that isolation and deenergization of the machine or equipment have been accomplished: Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
