29 CFR  1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:   a) 174 Woodmont Circle, River Vale, NJ:  An employee engaged in siding work on a two story residential house, was working from a pump jack scaffold approximately 15 feet above the ground without a guardrail system installed along the open sides,  on or about  8/12/2022.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.   a) 174 Woodmont Circle, River Vale, NJ:  An employee engaged in siding activities on a 6 on 12 pitched porch roof was exposed to fall hazards of approximately 15 ft to the ground below as a means of fall protection had not been utilized. Condition noted on or about  8/12/2022.   Vasquez Construction, Llc  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1537508, citation number 1, item number 1 and was affirmed as a final order on 09/7/2021, with respect to a workplace located at    Lot-#21, 265 E Crescent Ave, Mahwah, NJ 07430.  Vasquez Construction, Llc  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1385312, citation number 1, item number 1 and was affirmed as a final order on 10/27/2021, with respect to a workplace located at  Washington Street, Fort Lee, NJ 07024.
