An employee sustained a broken right femur when he fell approximately 14 feet from an extension ladder, on or about 8/12/2022.

On 8/17/2022 the employer provided the OSHA 300 log that did not have the employee who fell approximately 14 feet from an extension ladder and sustained bruises to both legs on 8/12/2022.
