29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs and traffic control devices at points of hazards: a. Site- On August 15, 2022 an employee of a subcontractor was injured when the articulating boom lift he was operating attempted to cross a bridge, that had previously been identified as structurally unsound for all but foot traffic, collapsed and plunged into a drained canal bed. The bridge was not posted with traffic signs or provided with traffic control devices that would prevent vehicles or construction equipment from accessing the bridge.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: Site- Employees and subcontractors on site were exposed to falls of approximately 11 feet when the sides of a drained canal that crossed through the jobsite were not provided with guardrails or other means of fall protection.
