29 CFR 1910.217(c)(2)(i)(a): Point of operation guard(s) on mechanical power press(es) did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guard(s)  a) On or about 08/17/2022 in the press department;  there was no point of operation guard on the Pivot Press while running in continuous mode exposing employees to amputation hazards.   b) On or about 08/30/2022 in the press department; where the point of operation guard on the 100 Ton Minster press had an opening of 2.25 inches wide between the guard and the frame of press and approximately 12 inches from the point of operation. This 2.25 inch X 12 inch gap on the left side of the press allowed access to the point of operation over, under and around the guard exposing employees to amputation hazards.   ABATEMENT DOCUMENTATION REQUIRED

29 CFR  1910.217(c)(3)(i)(c):The employer did not provide and ensure the point of operation device prevented the operator from inadvertently reaching into the point of operation at all times:  a) On or about 08/12/2022 in the press department; the point of operation device used for guarding (light curtain) the 100 Ton Bliss Press/Link Press was not adjusted to comply with Table O-10; a gap of 9 inches between the bottom of the light curtain and the bolster plate created exposure to the point of operation by reaching under the guard, exposing employees to amputation hazards.
