On or about August 18, 2022, at the address of 1501 Pray Blvd. Waterville, OH 43566: The employer did not ensure that each powered industrial truck operator successfully completed the training and evaluation, specified by OSHA PIV training standards prior to permitting employee(s) to operate powered industrial trucks (rough terrain forklift serial# LHO06603147). Failure to provide the required training and evaluation of PIV operators, exposed the operator, as well as others working in and around forklift operating areas to struck-by injuries, tipping, and rollover incidents.

On or about August 18, 2022, at the address of 1501 Pray Blvd. Waterville, OH 43566: The employer failed to ensure that a wooden job made scaffold attached to the forks of the rough terrain forklift serial# LHO06603147 which employees worked was designed by a qualified person and constructed/loaded in accordance with that design. Employees operating from this elevated scaffolding in this condition were exposed to falls of 28ft.

29 CFR  1926.451(c)(2)(v):Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  a)	Melanie Construction LLC. On or about August 18, 2022, at the address of 1501 Pray Blvd. Waterville, OH 43566: The employer did not ensure that a wooden job made scaffold platform used to hoist employees to higher levels was entirely attached to the forks of the rough terrain forklift serial# LHO06603147 and that the forklift was not moved horizontally while the platform was occupied. Employees using Scaffold in this condition as well as others working in and around forklift operating areas are exposed to struck-by injuries, fall injuries, tipping, and rollover incidents.

Employees operating in this condition were exposed to fall hazards from a fall of 28 feet or greater to the ground below.
