29 CFR  1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D).  At the facility located at 1997 American Boulevard, De Pere, WI  54115, the employer's machine specific energy control procedures for 55 Gallon Fill Line and 2.5 Gallon Fill Line did not identify all sources of potentially hazardous energy such as, but not limited to chemical and pneumatic, exposing employees to hazardous energies when employees perform servicing or maintenance work.

29 CFR  1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  At the facility located at 1997 American Boulevard, De Pere, WI 54115, the Ribbon Blender (SN 110-120-HD) was not adequately guarded to prevent employee access when adding materials, exposing employees to rotating parts.

29 CFR  1910.132(d)(1)(i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment.   At the facility located at 1997 American Boulevard, De Pere, WI 54115, 2.5 Gallon Fill Line, the employer did not select and require the use of personal protective equipment that will protect employees from the hazards identified while performing changeover, exposing employees to struck by chemical hazards. The employer did not require the use of chemical goggles or face-shield during changeover with Blue Foam Low pH Detergent.
