29 CFR  1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage and other deterioration, and defective components shall be removed from service.   181 Second Street Pittsfield, MA: On or about August 18, 2022 and at times and at times prior. The employer did not ensure that damaged personal fall protection equipment was removed from service. An employee was observed wearing a fall protection harness with a broken plastic chest strap adjuster, surface rust on adjustment buckles, the  label on fall protection harness was not intact and discoloration of the fall protection harness was observed.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.    181 Second Street Pittsfield, MA: On or about August 18, 2022 and at times prior, Employees were observed working on a residential structure engaged in roofing activities with unprotected sides and edges up to 20 feet above the next lower and were not protected by either guard rails, safety net systems or personal fall arrest systems exposing them to fall from height hazards.  J and Z Roofing and Construction, Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(b)(13) , which was contained in OSHA inspection number 1548293, citation number 1, item number 1 and was affirmed as a final order on 01/12/2022, with respect to a workplace located at 16 Kathy Way, Pittsfield, MA.
