29 CFR  1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:   189 First St., Pittsfield, MA: On or about December 22, 2025, the employer did not ensure that employees using personal fall arrest systems had their lanyards attached to their harness at a point in the center of the wearer's back, while working at heights of approximately 25 feet during a residential construction project involving shingle replacement.  J&Z Roofing & Construction Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.502(d)(17), which was contained in OSHA inspection number 1615745, citation number 1, item 1a, and was affirmed as a final order on November 30, 2022, with respect to a workplace located at 181 Second Street Pittsfield, MA.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  189 First St., Pittsfield, MA: On or about December 22, 2025, the employer had not prepared a written certification record of fall protection training which included the name or other identity of the person trained, the signature of the person trained, date(s) the training was completed and the signature of the person who conducted the training for employees performing shingle replacement work on a residential rooftop and exposed to fall hazards of approximately 25 feet.  J&Z Roofing & Construction Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(b)(1), which was contained in OSHA inspection number 1672802, citation number 2, item 1b, and was affirmed as a final order on September 29, 2023, with respect to a workplace located at 63 Maple Grove Drive, Pittsfield, MA.  J&Z Roofing & Construction Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(b)(1), which was contained in OSHA inspection number 1615745, citation number 1, item 3b, and was affirmed as a final order on November 30, 2022, with respect to a workplace located at 181 Second Street Pittsfield, MA.
