29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets:  a. On the lower roof section of a temporary residential new construction worksite, located at or near, 6991 Red Timber Road, Lot 37, in Jacksonville, Florida 32244: On or about August 17, 2022, the employer exposed an employee to head injury hazards caused by flying objects/debris, in that head protection was not utilized by the employee working beneath another employee, while they both installed roof underlayment type paper.

29 CFR  1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury:  a. On the roof of a temporary residential new construction worksite, at or near, 6991 Red Timber Road, Lot 37, in Jacksonville, Florida 32244: On or about August 17, 2022, the employer exposed three employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employee, while using pneumatic nail guns to install underlayment on the roof.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a.  On a 5:12 pitched roof of a temporary residential new construction worksite, at or near, 6991 Red Timber Road, Lot 37, in Jacksonville, Florida 32244: On or about August 17, 2022, the employer exposed three employees to fall hazards ranging from 6 to 26-foot, 6-inches, in that fall protection was not utilized by employees while installing underlayment paper.
