At 2323 North State Street, Bunnell, Florida: On or about January 20, 2023, the employer exposed an employee working from a JLG 8042 aerial lift to a 20-foot fall hazard, in that he was not wearing a harness and lanyard attached to the boom or basket.

At 2323 North State Street, Bunnell, Florida: On or about January 20, 2025, on the 1:12 pitch roof, the employer exposed employees performing leading edge work to a 20-foot fall hazard, in that they were not protected by use of a guardrail system, safety net system, personal fall arrest system or any alternative fall protection.
