29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment: a) Jobsite located at 8 East Oregon Avenue Suite R, Philadelphia, PA: Express did not verify that the workplace hazard assessment had been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment, for required use of PPE for workplace tasks. The host employer, Royal White Cement, provides a variety of personal protective equipment (PPE) for the employees to use, including but not limited to half-face respirators and gloves. Employees are exposed to chemical hazards when dispense and fill trucks with White Portland Cement Type I-II, a corrosive. Condition noted on or about June 1, 2022.
