29 CFR 1926.701(b):All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement: At job site located at U.S. Route 400, Mile Marker 191, Bridge overpass, Westbound U.S. Route 400 & KS-251, Cheney, KS 67025: Employees engaged in bridge construction activities when accessing the wooden platform on the east side of the bridge were exposed to struck-by hazards in that protruding steel rebar was not capped or guarded to eliminate the hazard of impalement. The wooden platform stood approximately 4.5 feet above the structure beneath where protruding rebar was present.

29 CFR 1910.134(c)(2)(i):Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (Construction Reference 1926.103) At job site located at U.S. Route 400, Mile Marker 191, Bridge overpass, Westbound U.S. Route 400 & KS-251, Cheney, KS 67025: Employees engaged in concrete sawing activities, during bridge construction activities, were exposed to respiratory hazards in that the information contained in Appendix D was not provided. If a respirator is used improperly or not kept clean, the respirator itself can become a hazard to the worker. Therefore, it is necessary to take certain precautions to be sure that the respirator itself does not present a hazard.
