29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: In the Fuller Gript area, employees conducting manufacturing activities using the American lathe, SN: FG-65, were exposed to amputation, struck-by and caught-in hazards. Machine guarding was not being used to protect the employees while using the American Lathe with an unguarded lathe chuck and unguarded rotating shafts with projecting set screws.

In the Fuller Gript area, employees engaged in material movement are exposed to struck-by hazards in that the employer did not ensure slings that are missing affixed and legible identification markings were removed from service. In the Fuller Grip area, the following synthetic slings were found to be missing identification markings: A) Two type VI return eye 6' by 12" synthetic web sling B) One type III flat eye 4' synthetic web sling C) One type V endless 5' synthetic web sling
