Located at 8108 Rapala Lane, Pickerington, Ohio 43147: On or about August 24, 2022, the employer did not maintain an effective safety and health program to ensure that fall and struck by hazards were identified and corrected onsite while employees performed framing work.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. Located at 8108 Rapala Lane, Pickerington, Ohio 43147: On or about August 24, 2022, employees were preforming framing work from the first and second story roofs without fall protection, thereby exposing the employee to fall heights from approximately twenty feet.
