29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: At a worksite located at 1515 W 9th Street, Kansas City, MO 64101: Employees were exposed to fire, and electrical shock hazards due to the electrical extension cord set being plugged into another set of extension cords in series and not per the listing, labeling, and certification of the extension cords. On or about August 23, 2022, extension cords, and three-way extension outlets were being used by workers to power microwaves, portable fans, and other too equipment.
