Location: Warehouse, 1 New England Way, Ayer, MA.   a) On or about 8/24/22, the employer did not ensure that steps ascended by an employee had uniform riser heights. An employee slipped while ascending from the first step, approximately 14.25 inches from the initial level surface, to the second step, at approximately 31.75 inches from the initial level surface.  b) On or about 8/30/22, the employer did not ensure that steps as a part of a conveyor crossover walkway had uniform riser heights. The riser height for the initial and top steps was approximately 11 inches, however the middle step was approximately 13 inches in height.
