29 CFR 1910.25(b)(3): The employer did not ensure that stairs have uniform riser heights and tread depths between landings. Location: Warehouse, 1 New England Way, Ayer, MA. a) On or about 8/24/22, the employer did not ensure that steps ascended by an employee had uniform riser heights. An employee slipped while ascending from the first step, approximately 14.25 inches from the initial level surface, to the second step, at approximately 31.75 inches from the initial level surface. b) On or about 8/30/22, the employer did not ensure that steps as a part of a conveyor crossover walkway had uniform riser heights. The riser height for the initial and top steps was approximately 11 inches, however the middle step was approximately 13 inches in height.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: Location: Warehouse, 1 New England Way, Ayer, MA. a) On or about 8/30/22, a walkway over a conveyor system did not have adequate protection to stop pallets from moving on the conveyor and striking employees, as employees cross over to an industrial vehicle.
