29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:       a)  At Lots 148 - 155 Raiden Circle, The Borough at Wyndham South, Opelika, Alabama:  On August 31, 2022; the employer failed to ensure employees using multiple extension ladders on site to gain access to the roof had the rails of the ladders extended at least three feet above the roof landing to provide safe access  exposing his employees to a fall hazard of up 20 feet  3 inches to the ground below.

29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.   a)  At Lots 148 - 155 Raiden Circle, The Borough at Wyndham South, Opelika, Alabama:  On or about August 31, 2022 and at times prior; the employer allowed his employees to operate pneumatic nail guns without eye protection while performing roofing activities exposing his employees to struck-by hazards.  The Alex Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1501085, citation number 1, item number 1 and was affirmed as a final order on February 1, 2021 with respect to a workplace located at Dingo Drive, Opelika, Alabama and  in OSHA inspection number 1563918, citation number 2, item number 1 and was affirmed as a final order on January 13, 2022 with respect to a workplace located at 2142 Seqoia Dr. Lot 268 The Preserve, Auburn, Alabama.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At Lots 148 - 155 Raiden Circle, The Borough at Wyndham South, Opelika, Alabama:  On August 31, 2022; the employer failed to ensure employees involved in roofing activities were using fall protection exposing his employees to a fall hazard of up 32 feet  to the ground below.  The Alex Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1501085, citation number 1, item number 2 and was affirmed as a final order on February 1, 2021 with respect to a workplace located at Dingo Drive, Opelika, Alabama and in OSHA inspection number 1563918, citation number 2, item number 2 and was affirmed as a final order on January 13, 2022 with respect to a workplace located at 2142 Seqoia Dr. Lot 268 The Preserve, Auburn, Alabama.

The Alex Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.503(a)(1), which was contained in OSHA inspection number 1563918, citation number 1, item number 1 and was affirmed as a final order on January 13, 2022 with respect to a workplace located at 2142 Seqoia Dr. Lot 268 The Preserve, Auburn, Alabama.
