29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  On or about August 1, 2023 the employer Alex Roofing LLC at jobsite located at 3330 Liberty Ave. Vermilion OH 44089, did not ensure that each affected employee used appropriate eye or face protection when performing roofing work on a existing residential structure while exposed to potential struck by hazards (flying particles/debris) while using pneumatic nail guns.

29 CFR  1926.501(b)(11):Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about August 1, 2023 the employer Alex Roofing LLC at jobsite located at 3330 Liberty Ave. Vermilion OH 44089, did not ensure employees were protected from falls while conducting commercial roofing construction activities on the second (roof) level of a commercial building, while exposed to a fall of approximately 25 feet or greater to the ground below. Employees were working atop a steep roof with a 14:12 pitch at the peak of the roof.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about August 1, 2023 the employer Alex Roofing LLC at jobsite located at 3330 Liberty Ave. Vermilion OH 44089, did not ensure the side rails of one portable Keller brand aluminum KPRO 17FT IA Multi-purpose KMT-17 ladder was extended at least three feet above the landing surface. Nine employees were exposed to falls from approximately 9 feet or greater from the top of the second (roof) level to the decking below while using the ladder to access the second (roof) level to perform commercial roofing activities on a commercial building.
