On the roof of a residence with a 6:12 pitch, at a temporary worksite located at 424 Caribbean Place in St. Johns, Florida: On or about September 1, 2022, the employer exposed employees to fall hazards ranging from 6 to 11-foot, in that fall protection was not utilized by employees while performing roofing work.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a. At a temporary residential worksite, located at 424 Caribbean Place in St. Johns, Florida: On or about September 1, 2022, the employer exposed employees to fall hazards ranging from 6 to 11-foot, in that the side rails of a potable ladder were not extended at least 3 feet above the roof (upper landing), while employees ascended and descended the ladder for access to the roof work area.
